Sheffield City Council, GB Team Around the Person Adult Care

City officials identified obstacles to effective care for residents with multiple health needs. Residents referred to this service are assigned a Team Around the Person (TAP) coordinator. The coordinator works with each patient, their support network, and community support workers on a personalized care plan. TAP coordinators conduct informal huddles with local specialists and general practitioners for expert guidance during plan development.
