Project Succeeded

Over 800 validated resources have been incorporated into the CAREAPP directory. 29 community organizations and 850 providers have been trained to use the platform. CAREAPP has improved effectiveness of coordination with over 13,000 searches and 1,500 online referrals. CAREAPP decreases staff time searching for resources from 120 minutes to only 5 minutes with an 80% user satisfaction rate.
