Wexford County Council, IE Telehealth Chronic Health Pilot

A multi-agency project team recruited 50 patients in Wexford County for a 12-week pilot. This recruitment focused on patients with chronic health failure, COPD, and diabetes. Participants used Tunstall's myMobile app for self-guided daily collection of blood pressure and oxygen levels. Care providers remotely monitored and responded to condition changes with the triagemanager platform. Each patient received medical equipment based on their current condition.
