Council-owned facilities work with general practitioners and social service providers on comprehensive care for residents. Customized plans allow carers to serve residents based on their needs and goals rather than one-size-fits-all checklists. The Home Care Project contracts private specialists in areas not served by the county for evenly distributed care. Project partner Betsi Cadwaladr University Health Board helps with program evaluation and specialist recruitment.
