Caledon, ON COVID-19 Mobile Screening Survey

The COVID-19 Screening Questionnaire asks if the user is an employee or visitor to activate different paths. Additional self-screening questions about symptoms, recent travel, and planned building visits must be answered before each visitor entry. Survey responses are automatically sent to building personnel, replacing in-person interviews. Caledon's IT team developed the survey to work on desktop and mobile devices with data protections to secure user privacy.
