Toronto, ON Community Crisis Support Service Pilot

Service providers were selected for neighborhood-level interventions in four pilot zones. Each provider operates an around-the-clock response team of social workers, counselors, and nurses. Participating organizations handle case management from initial intake through follow-up evaluations. Non-emergency crises are identified through a partnership between the Toronto Police Service and Findhelp|211 Central. Pilot results will shape citywide adoption of this model by 2025.
