Turlock, CA Person-Centered Care Project Pilot

The Turlock City Council approved $200,000 for a one-year pilot with the community health organization. The Person-Centered Care Project reduces hospitalizations and nursing home admissions through in-home services. The pilot targets residents aged 55 and older and disabled residents who don't qualify for the MediCal program. Eligible residents receive free services including palliative care from Community Care Choices and non-medical support from CareLinx.
